This application should be completed by those wanting to acquire a Class D Contractor's Licence issued pursuant to Sections 5 - 8 of The Passenger and Freight Elevator Regulations, 2017.

Class D authorizes the holder to:
(i) Provide elevator safety training programs and instruction to elevator emergency personnel, including those persons licensed as a Class B - Owner; or
(ii) access the elevator equipment areas such as the hoistway, top of car, pit areas, machine rooms and control rooms for the purpose of conducting a survey, audit, or developing elevator specifications on behalf of the elevator owner

To be eligible for registration for Class D licence, please provide the following:
1) Number of years the company has been engaged in the field as an elevator consultant firm in other jurisdictions;
2) Number of years in which the company has provided elevator consulting services for installations in Saskatchewan; and
3) List of employees, training and experience relating to the elevator industry safe work practices and procedures for accessing all elevator areas for the purpose of inspection and survey.

• His/her company when registered as a consultant D (contractor) will comply with all requirements of The Passenger and Freight Elevator Act and The Passenger and Freight Elevator Regulations
• His/her company will ensure that all mechanics or engineers have access to the latest edition and revision of the code(s) applicable to the elevating devices on which they are assigned to work and that they will have full working knowledge of such codes
• Applicant has self ensured that supervisory staff, listed mechanics or engineers have full knowledge of the Technical Safety Authority of Saskatchewan Act, The Passenger and Freight Elevator Act and The Passenger and Freight Elevator Regulations
