Submission Prerequisites
IMPORTANT: To avoid processing delays, prior to completing this application, please ensure a copy of the QCP manual has been emailed to qualityprograms@tsask.ca for review:

Please ensure this form is complete and correct, prior to submission with the applicable fee of $310.00 for each manual to be revised.

The breakdown of your $310.00 is as follows:

- Base Fee for Review - includes up to 2 hours of service.

- Replacement of certificate/licence

Be advised that additional expenses associated with this service may be incurred and will be invoiced upon completion of review.

The Quality Program registration process consists of manual review and audit. The audit shall be accommodated within six months following completion of the manual review. If the audit is not accommodated within six months a new application and payment will be required.
